Sample assessment worksheet for the skill of accessing valid and reliable resources

This is an excerpt from Lesson Planning for Skills-Based Secondary Health Education 2nd Edition With Web Resource by Sarah Benes,Holly Alperin.

Standard 3 Assessment Worksheet: Accessing Valid and Reliable Resources. You have been volunteering with a local mental health support organization in our community. One day, after telling your supervisor about the accessing valid and reliable resources unit you were doing in health education, your supervisor suggested that you create content for a new page on their website geared toward teens. They want you to create content that relates to recognizing the need for mental health support, getting help for oneself or others, supporting mental well-being, sharing resources, and breaking down barriers and stigma related to mental health.
For this project, you need to do the following:
Create a scenario that includes signs and symptoms of mental health conditions or concerns.
Identify the signs and symptoms that were present in the scenario.
Provide at least two valid and reliable resources that teens in the community could go to for help.
Develop ACCESS evaluations for each of the resources you identify.
Provide sample phrases to say when accessing help, things to be prepared for, and reasons to get help even if it feels hard.
Create content related to encouraging someone to take action to support their mental well-being. Include benefits (with sources), strategies, and at least one resource that could provide help or support. Include ACCESS evaluations for each resource you identify. Standard 3 Assessment Rubric: Accessing Valid and Reliable Resources. Visit the web resource to access supplementary materials.
